• This is a fixed fee promotion. If you choose to decline some of the services included in this promotion, the fee will not be changed.
• If you have a private health insurance your insurance must pay a minimum of $320.00 for this service, if your insurance does not cover this amount you will be responsible for any fees up to this amount
• Any additional amount paid by your health insurance is dependent on your insurance fees and is not reimbursable to you, in cash or as a credit, or to your fund.
• For patients taking up this offer, our clinicians will aim to remove existing dental stains. However, for some patients general professional cleaning and polishing may not be enough to achieve removal. Additional or alternative treatments may be recommended to address the staining concerns.

Our Complimentary Smile Consultation includes a 30-minute consultation appointment under ADA Code 014 to discuss your smile goals, treatment options, and provide a preliminary overview of possible treatment pathways.

Please note the following terms:

• The complimentary consultation applies only to ADA Code 014 and is limited to a maximum duration of 30 minutes.
• To provide an accurate treatment plan and quotation, a separate Scan & Records Appointment is required prior to commencing treatment. This appointment is charged at $280 and may be completed on the same day as the consultation or booked for another date.
• The Scan & Records Appointment may include photographs, digital scans, radiographs, impressions, or other diagnostic records as clinically required.
• Any treatment, examination, radiographs, diagnostic procedures, or additional ADA item codes performed outside of ADA Code 014 during the consultation appointment will incur additional fees.
• If the consultation extends beyond the allocated 30 minutes, additional time will be charged at the standard consultation fee of $80.
• Complimentary consultations are intended for discussion purposes only and do not constitute a final treatment plan, diagnosis, or fixed quotation until all required records and assessments have been completed.
